Driver's Application for Employment
A standardized FMCSA‑compliant application used to capture the driver’s full legal name, address, employment history, license details, accident record, and safety‑sensitive experience. Required for all new driver hires as part of the qualification process.

Certificate of Driver Compliance & Road Test
Documents the driver’s successful completion of a road test or CDL exemption in accordance with FMCSA § 391.31 or § 391.33. Also includes a signed statement of qualification and regulatory compliance.

Safety Performance History Record Request
Sent to the driver’s previous employers to retrieve records of crashes, safety violations, terminations, and substance abuse issues over the past three years. This FMCSA‑mandated form ensures safety performance transparency.

Certification of Violations & Annual Review of Driving Record
Documents all traffic convictions over the past 12 months and confirms the carrier has reviewed the driver’s MVR (Motor Vehicle Record) annually. Required to be completed and retained in the DQF per § 391.25 and § 391.27.

General Consent for Limited Queries of the FMCSA Drug and Alcohol Clearinghouse
Authorizes carriers to conduct required annual limited queries through the FMCSA Clearinghouse to check for drug and alcohol violations. Compliance with CFR 49 § 382.701 is mandatory for all CDL drivers.

Lease Agreement
Outlines the legal agreement between the carrier and driver regarding the leasing of equipment (truck, trailer, etc.). If applicable, this document must be included in the DQF to reflect the working relationship and equipment use.

Verification of Medical Examiner’s National Registry Number
Proof that the driver’s physical was performed by a certified medical examiner listed on the National Registry of Certified Medical Examiners, as required under CFR 49 § 382.701. Ensures the validity of the medical certificate.

DQF Checklist
An internal quality control checklist verifying that all required forms, signatures, and supporting documents are present and up to date. Ensures nothing is missed before audits or FMCSA inspections.
