A standardized FMCSA‑compliant application used to capture the driver’s full legal name, address, employment history, license details, accident record, and safety‑sensitive experience. Required for all new driver hires as part of the qualification process.

A signed consent form authorizing the carrier to obtain background checks, verify past employment, contact previous employers, and review driver safety and accident history. Required to initiate any DOT‑compliant pre‑employment screening process.

Grants permission to access the FMCSA’s Pre‑Employment Screening Program (PSP), which reports a driver’s 5‑year crash and 3‑year inspection history. Helps carriers assess a candidate’s past performance before making a hiring decision.

Documents the driver’s successful completion of a road test or CDL exemption in accordance with FMCSA § 391.31 or § 391.33. Also includes a signed statement of qualification and regulatory compliance.

Sent to the driver’s previous employers to retrieve records of crashes, safety violations, terminations, and substance abuse issues over the past three years. This FMCSA‑mandated form ensures safety performance transparency.

Documents all traffic convictions over the past 12 months and confirms the carrier has reviewed the driver’s MVR (Motor Vehicle Record) annually. Required to be completed and retained in the DQF per § 391.25 and § 391.27.
